How to Begin Your Care Journey
To utilize these benefits, start with a Personal Care Assistant (PCA) assessment to determine your needs. An assessor will visit your home to review your daily activities and health status. To request this assessment, simply contact your local county agency or your health plan member services.
Personal Care Assistant (PCA) Services
Our dedicated PCAs at Diamonds Home Health Care, Inc. provide essential support for activities of daily living (ADLs) and health-related tasks. Here’s how we can assist:
