If you are enrolled in Medical Assistance (MA), MinnesotaCare expanded benefits, Alternative Care Program, or a waiver program, you may qualify for services paid through the county of your residence. You must have an evaluation of your need to see if PCA or similar services are right for you. An assessor visits your home and reviews your daily needs and health. To request a PCA assessment, you must contact your local county agency or health plan member services to see if you are eligible.

Personal Care Assistant (PCA) Services

The PCA performs day to day activities in the home and community. Our PCA’s at DIAMONDS HOME HEALTH CARE INC assist our clients with their activities of daily living, health-related procedures, and tasks, observation, and redirection of behaviors.

Our PCA worker may assist the clients with the following ADLs:

Dressing

Including the application of clothing and special appliances or wraps.

Grooming

Including basic hair care, oral care, shaving, basic nail care, applying cosmetics and deodorant, care of eyeglasses, and hearing aids.

Bathing

Including basic personal hygiene and skin care.

Eating

Including completing the process of eating, including hand washing and application of orthotics required for eating, transfers, and feeding.

Transfers

Including assistance to transfer the person from one seating or reclining area to another.

Mobility

Including assistance with ambulation, including use of a wheelchair; not including providing transportation.

Positioning

Including assistance with positioning or turning a person for necessary care and comfort.

Toileting

Including helping persons with bowel or bladder elimination and care. This includes transfers, mobility, positioning, feminine hygiene, use of toileting equipment or supplies, cleansing the perineal area, an inspection of the skin, and adjusting clothing.

Instrumental activities of daily living (IADLs)

Accompany to medical appointments, accompany in the community, assist with communication, and shopping for essential items.

Homemaking/Home Management Services

We provide services that help clients manage general cleaning and household activities.

Our services include:

  • Cleaning, home management, or assistance with ADL’s
  • Light housekeeping tasks
  • Laundry services
  • Meal planning, shopping, and preparation
  • Shopping for household essentials
  • Assist with ADL’s as needed

Individual Community Living Support Services (ICLS)

We aid and support clients who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own homes. Our ICLS assistance and support is for eligible people; 65 years and older, who use Elderly Waiver (EW) or Alternative Care (AC) programs.

Our services include:

  • Active cognitive support (problem-solving, assurance, redirection)
  • Adaptive support service
  • Activities of daily living
  • Household management
  • Health, safety, and wellness
  • Community engagement

Companion Services

These are non-medical care, supervision, and socialization provided to a person age 18 or older. These services help clients work towards a therapeutic or community integration goal in their support plan.

We keep you company with activities like:

  • Attending a movie to practice coping skills to manage social anxiety.
  • Attending a community event to reduce their social isolation.
  • Playing a board game to enhance your fine motor skills.

Night Supervision

Overnight assistance monitoring is provided by awake staff in the client’s own home.

Our services include:

  • Carrying out the positive support plan
  • Reinforcing independent living skills training

Respite Services

We provide families and caregivers with a much-needed break while offering a safe environment for their children. Our respite care service gives families a chance to re-energize and refocus.
